請下載住宿申請表並雙面列印,由法定監護人於正反兩面簽名。
Please download and print the Dormitory Application Form double-sided. Both sides must be signed by the student’s legal guardian.

第一步|完成線上申請
Step 1 | Complete the Online Submission
請完成以下兩項:

- 填寫住宿申請基本資料(Google Form)。
Complete the Dormitory Applicant Information Form (Google Form). - 上傳已完成法定監護人於正反兩面簽名之住宿申請表(Google Form)。
Upload the completed Dormitory Application Form signed by the student’s legal guardian (Google Form)
第二步|掛號郵寄住宿申請表正本
Step 2 | Mail the Original Application by Registered Mail
請將已完成法定監護人於正反兩面簽名之住宿申請表正本,以掛號方式寄送至下列地址。掛號寄出後,請將掛號收執聯拍照,並使用申請時所使用的主要電子郵件信箱,寄送至下列兩個電子郵件信箱,以利本校確認收件。

After mailing the document, please take a photo of the registered mail receipt and send it from the primary email address used for the admission application to the two email addresses listed below for our confirmation.

請務必完成上述兩個步驟(線上申請及掛號郵寄正本),始完成住宿申請程序。
Your dormitory application will be considered complete only after both steps (online submission and registered mailing of the original application) have been completed.
注意事項
Important Notes
- 宿舍不提供早餐,學生可至校內 OK 便利站購買。
Breakfast is not provided. Students may purchase breakfast at the on-campus OK convenience store. - 申請前請確認學生能遵守宿舍規定及作息(包含每日公共區域及寢室清潔)。
Please ensure that the student is able to comply with all dormitory rules and daily routines, including daily cleaning responsibilities, before applying. - 宿舍床位有限,提出申請並不代表保證入住。
Dormitory space is limited. Submission of an application does not guarantee a dormitory placement. - 住宿錄取名單將於開學前一週公告於學校網站,敬請留意相關公告。
The dormitory assignment list will be announced on the school website one week before the start of the school year. Please check the school website regularly for updates.
