Dear Parents,

We would like to inform you about an important update from the Taichung City Government regarding the Influenza Electronic System (CIVS) and the “Campus Influenza Vaccine Consent Form” for our G1 to G12 students.

The purpose of this consent form is to determine whether you agree to have your child receive the influenza vaccine. In accordance with this new policy, our school will no longer be providing paper forms for parental signatures. Please follow the instructions below to complete the online vaccine consent form：

1. Please click on the provided link using a mobile phone to initiate the online signing process. When signing, use your regular handwriting to replicate your signature.
2. Regardless of whether you intend for your child to receive the influenza vaccine, it is imperative that you complete the online signing process before November 8th.
3. It is important to note that you can modify your consent online at any time before the vaccine is administered.
4. Our school has scheduled the administration of the influenza vaccine for November 14th (Tue).

Please be aware that the system provided by the Ministry of Health is currently available only in Chinese. Despite our efforts to inquire with the Ministry of Health, there will not be an English version available in the near future.
